Year 12 HSC Exams - Protocols and Procedures
Wollongong Catholic Education Office requires all its Diocesan Secondary Schools to collect information about the health of all HSC students attending exams.
A signed COVID-19 HSC Exam Student Declaration Form must be completed and provided to a staff member before entering every exam. A copy of the form is attached. Copies will also be available at the College, outside CAPA before each exam. Students are required to print and complete a separate form for each exam, which must be provided before entering the exam room.
Students are encouraged to do this prior to arriving at the College, to help expedite the process.
If a student has an ongoing medical condition that could be mistaken for flu-like symptoms, they must provide written assurance from their doctor or GP detailing the symptoms they are displaying and confirming they are of a known medical condition and are normal for the student. The written assurance should be provided to the school prior to the start of the exam period.
